In AIcares, we are combining AI-driven telemonitoring with specialised home care environments to improve the management of heart failure patients. Telemonitoring, which enables the wireless transmission of patient health data for remote management, is becoming increasingly common around the world. This proactive approach reduces the need for hospitalisation, in line with the digital transformation of global healthcare. Patients with advanced heart failure often require recurrent hospitalisation, posing significant challenges for both patients and healthcare systems.
We aim to develop a predictive AI-driven model to identify heart failure patients at high risk of mortality within one year by analysing electronic health records. We will also implement an intuitive interface using language models to communicate predictive results to healthcare professionals to facilitate their decision-making.
Our goal is to provide cost-effective, personalised and safe care for patients with advanced heart failure. By combining telemonitoring, specialised home healthcare and AI-based predictive modelling, AIcares has great potential to both improve patient outcomes and reduce healthcare costs.
